There is a discussion unfolding on facebook right now and I see one question that keeps on popping up from DM patients. It would be great if someone like Afrezzauser could explain, with the credibility of first hand experience, that the pk profile negates the need for precise dosing.

Julian Hermanson Dilorenzo How come just 4 and 8 units? That's not very specific Type 1 Diabetics can require as little as less than a unit to more than 10 units for some depending on the blood sugar and food or amount of Carbs being eaten. Plus when will this be an option in USA ?how come doctors don't push these new products if they are FDA approved already? It's been about a year or so right since this Inhaler has been attempted... I really want this if it works!!!

Hello I am a type 1 for about 18 years. My research in Afrezza and my opinion is the reason for the 4 unit and 8 unit is not for precise dosing but more like for small and large meals and most likely for whatever level your BG is at. Taking Humalog you need carb counting because the insulin doesn't act as fast as Afrezza. Lets say your reading is 160 before lunch and you want to dose for that meal using Humalog.. You count carbs and calculate how high your BG is going to rise from 160 to lets say 210? then the dosing also measures where you want to come back down to lets say 120.. not to mention the roller coaster ride after 2 hours to get there! Afrezza works 12-15 after inhaling it so you can have a reading at 160 and chances are if you take a 4 unit right before a meal you can be sitting at a comfortable level of maybe 115 or lower within 35 minutes. The key is what you eat, what your bg level is at the time you eat and where you want to be afterwards. If you are eating just a protein based meal with minimal carbs and your reading is 110. I suggest not taking afrezza until about 15 minutes after you eat. Protein in my experience and I am sure you all know this of course doesn't raise blood sugars nearly as fast as carbs do. My CGM has helped me tremendously to monitor patterns of certain foods. Not everyone is made up of the same chemistry. For instance, some people can eat pretzels and not have a spike where if I were to eat a handful of pretzels, my bg would go through the roof!. Bottom line is Humalog and novalog (so called fast acting insulins) aren't great for someone wanting to have a great A1c levels. That lag of time that your bg spikes throughout the day after meals and the time it takes trying to bring it back to normal can really have an effect on your A1c not to mention that those rollercoaster rides and spikes are bad for your eyes, nerves, etc. Sorry to sound repetitive!
