At this stage a lot of the early adopters will be patients who have had some prior knowledge of Afrezza. But as more and more users find out Afrezza, you will see more people who got introduced to Afrezza by other users or their friends. I believe Eric has introduced Afrezza to some friends and they started to use them. Wouldn't surprise me if up to 3/4 of the patients now on it were in the trials. Saddens me. But wouldnt surprise me

One thing that Exubera could and probably does help Afrezza is that overtime, substantially, if not all, of the satisfied patients of Exubera will be patients of Afrezza. While there are not so many, there are still some out there. If I recall correctly, Exubera has all together 80,000 prescriptions and 24,000 patients. Not sure those numbers were for US or globally, but I would guess we should at least catch most of these patients in the US to the extent they liked inhaled insulin. That will help at least during the early stages of the launch.

Doctors will be hesitant to prescribe Afrezza to new patients until they see actual lab reports from the early users. During the next 4 weeks over 600 of the early Afrezza users should start getting followup lab tests and then an additional 200 or so every additional week. Spiro's will be on May 27. Brian Sharp just reported a 5.7, down from 10.1. How can Doctor's ignore results like that? Spiro here, depressed in Miami. I wonder if depression can raise blood sugars? After 7 weeks on Afrezza and was taken off Actos but still on Metformin, a1c decrease to 6.2 from 6.5. My doctor wants test every 60 days until he get comfortable. Happy to report on side effect, I lost 11 lbs. Frankly, I was looking for under 6 but given I rarely have hypers now (bg>250) and I can sense it especially with my feet, I'm feeling great. I'll post a more detail report on my progress on another thread when I get a chance perhaps this weekend.

It is a pity that sales pick up so slowly, but in the long run it just doesn't matter. Real life results appear to be nothing short of spectacular. Word of mouth will take care of things. I'm looking at it like a pyramid scheme. Assume there are roughly 2000 patients today. If on average every new patient convinces 2 others in the 3 months after they start we are looking at something like this:
00: 2000 03: 2000 + 4000 = 6000 06: 2000 + 4000 + 8000 = 14000 09: 2000 + 4000 + 8000 + 16000 = 30000 12: 2000 + 4000 + 8000 + 16000 + 32000 = 62000 15: 2000 + 4000 + 8000 + 16000 + 32000 + 64000 = 126000 18: 254000 21: 510000 24: 1022000 27: ...
So growth can remain slow for the rest of the year, but at some point it will like an avalanche. Obviously a pyramid scheme is not sustainable, but there is a large supply of diabetes patients out there.
That is one effect. We must also assume that endo's and doctors are not stupid. Once they are convinced by the results of their first patients, they will move all their patients to Afrezza. The multiplier effect of that is huge.

Things that work well are successful in the long run. We would like to think so, but this staement is a perfect example of "survivorship bias". Who would know about the things that work well and did not succeed?

Things that work well with no competition. There really is nothing else in the market now that can compare to Afrezza. In the long run I can't see this not being successful.
