The formulary data tables below are presented in a format:

% of Lives Preferred (Unrestricted) = Preferred Drug with no restriction
% of Lives with Unrestricted Coverage = Coverage with no restriction
% of Lives with Covered (PA/ST) = Coverage but Prior Authorization or Step Therapy restrictions apply
% of Lives with Preferred (PA/ST) = Preferred Coverage but Prior Authorization or Step Therapy restrictions apply
% of Lives Not Covered = No Coverage
All levels are accompanied by +/- = Changes in Coverage from previous quarter

Formulary plans are listed for five channels:

A separate table is posted for each formulary channel and the tables will be updated QUARTERLY (Effective 2nd Quarter 2019).
