On ST.....$MNKD Kaiser Permanente: Afrezza is Tier 4 on Formulary, Available by Mail Order. Diabetics may want to change their plans to Kaiser Perma. Bullish Kaiser (So Cal) will not move the needle much if any. If there is a tier 1 generic drug, only a KP specialist is allowed to prescribe non generics. First line insulin rx is humilin N or a 70/30 combo for T2. So a patient would need to fail with humilin, probably then fail with lantus and meal time, then go on Afrezza. The amount of appointments so that my mom could get a pen instead of syringes was mind numbing. We qualified her because of arthritis, and they still pushed for the pharmacy to prefill syrniges. For my parents to get an exception to use pens, the endo had to sign off. Don't hold your breath on this one.

Par for the course with Kaiser I am afraid. I had 2 friends die of colon CA while with Kaiser (one early 30's and 1 late 40's). In both cases they diagnosed them with stomach ulcers and refused to consider colon CA. (both had family hx of colon CA and both asked the Dr for a colonoscopy at my urging). By the time they finally allowed for colonoscopies, the disease had already metastasized. One family has won a large law suit and the second has started a law suit. I would quit my company if they changed to Kaiser. What part of I had a parent die of colon CA at a young age and I have blood coming out both ends does not SCREAM "I need a colonoscopy"? Why either of them had not been having colonoscopies to begin with just stunned me.

I had an exploratory colonoscopy when I was 24 or 25, long family history of cancer, Aetna paid for it no questions asked found large polyps so I had to get one annually until they found no polyps also in 2017 Aetna started 0$ copay for generics if filled at cvs.

Clearly they wanted to cut costs and the easiest way to do that was to dump Mannkind and Novo Nordisk while retaining Sanofi and Lilly in exchange for rebates. I don't think that the pre-authorization for Afrezza will help either since it's not even a PA option (previously it was unrestricted) any more.

At this point in time Afrezza does not need to be on the same tier as Novolog/Humalog. We don’t need to offer the massive rebates to insurance companies during our relaunch phase. However, when an office does the required prior authorization (additional paper work, additional time for the office staff and doc to explain why the doc wants Afrezza instead of preferred agent) Afrezza needs to be Approved. Unfortunately, after the office and doctor performs this PA, many times Afrezza is still not approved. This needs to change in the first half of 2018 for Afrezza to grow affectively. I hope it does.

If offices were the decision makers on reimbursement yes. However, that lies with the insurance companies.

The way it is supposed to work is the insurance companies tire of the extra work they are having to do from the offices “pushing through” these scripts and ease their stipulations.

From MC radio interview, he mentioned BIC expansion in 2018. B will have obtained A approval in 2018. From other sources, Can and Mex will likely have A filed in 2018 by Mnkd. ME expansion including UAE, Saudi Arabia, Oman are also expected in 2018, probably combined with I. Perhaps Mgmt will begin to include A sales projections in some of those countries in 4Q18. It would be interesting by this time next year, 12/30/18, We will no longer be looking only at Symphony data. OS will have his work cut out for him trying to project A sales too.
