Bottom line was my endo was vehemently against placing a hormone into the lungs. Injection discomfort < potential damage to lungs. Your endo seems ill informed at the kinetics of inhaled monomeric insulin (Afrezza) vs sc hexameric insulin (all the rest). That in itself should make you wonder if the MD taking care if you had the knowledge base to guide diabetic patients well. Being a cautious conservative is fine. Being uninformed and calling it being conservative is often a red flag of uninterested and somewhat lazy MDs. The way you state his logic sounds more like a one liner pedaled by competitors of Sanofi/ MNKD then an informed scientific analysis I would expect from a big group of physicians? Can someone confirm that Kaiser (10 million covered in Ca) will not provide Afrezza as part of their coverage? If this is true , not good news. Any word on other big states (Texas) carrying Afrezza this year?

For what it's worth I did find Afrezza listed as Kaiser formulary on the lexicom site . It was dated 1/1/15 for both Northern & Southern Cali ( maybe too soon to show up on link posted on previous page?) . I even had our pharmacist look as well . I did a search at the top of the Lexicom page for Afrezza and it popped up . I wasn't misleading anyone . And if what I saw was incorrect I apologize . I would think paying extra to get Afrezza in the hands of patients on the front end would save them $ on the back end if it can help reduce hospitalizations due to complications associated with hypos, non compliance ect . I will be disappointed if it won't be covered after all . Unfortunately in my specialty I don't come across many Endos but should I run into one I'd love to hear what they have to say about Afrezza . At least we're having a Green day

It was more than just a one line response.
The doc talked about how adcom came up with their decision was flawed. Also how multiple cartridges for T2's would be very inconvenient, which I rebutted that in 2015 larger cartridges will be available. He went into how the docs colleagues were also against in at their department conference, which I really can't dispute. This of course is one small example in a very large pool of HMO's / PPO's.

As for the pay out of pocket - very common for companies (e.g. SNY) this early on to have their own special copay program to offset the fact that its not covered, or not enough yet. I got that from Genetech for Xolair and it made it cheaper even than some actual covered, preferred brand name drugs.
That said, I doubt they'll need to go down this path based on the need for a controlled launch, the market size, the initial production capacity and how many carriers are already covering at some level .... but they very, very easily could.

I just checked CareMark which is the recurring prescription drug supplier my insurance (United Health Care) has us use. I did a search and Afrezza came up in the search criteria (pre-populated) but when i executed the search it came back w/ nothing found. Odd.

JPG, Liane is correct , click on pic & it will rotate & you can enlarge . I'm able to do it from my IPhone . You all have helped me more than you know so anything helpful that I can contribute I'm happy to do BD was helpful instructing me how to post the pic , I should have done it sooner but I had no idea how to do so .
