Spiro, can you elaborate on your thought process regarding info from insurance versus MNKD/SNY? Mike, I'm pretty sure Spiro meant to ask why we "aren't" getting more information from the companies regarding the status of insurance coverage. I need to bio/pharma.. is it SOP to hear form insurance rather than companies?

Glad it is on Medicare Part D, but that still does not show in their formulary for Socal. jmp.sh/qZTWLthMy understanding is that when a drug is a PAB (Prior Authorization of Benefit, as Anthem calls it), it may not necessarily show up on their formulary, although I realize that doesn't make great sense. It's a PAB drug with Anthem, and that's what was originally posted on this thread, that is the Approval Criteria. I actually spoke to someone quite knowledgeable about Anthem's formularies, and it was unclear how it's a PAB, and part of the formulary, but may not be listed within the formulary. A bit like Schrodinger's Cat, unfortunately.
