Spiro’s prescription is for 24 units per day of 4 unit doses or 2 boxes per month. His insurance co-pay is $50 for the 2 boxes. BTW, it would be $50 if his prescription were for 1 box also.
Spiro believes that using Afrezza may require some adjustments in the eating habits of some diabetics. Simply put, no more 100 - 300 carb gram meals. Spiro really misses those days, but getting his blood sugar under control certainly trumps his cravings. Spiro tries to eat less than 30 grams of carbs at breakfast and lunch. At dinner, his goal is 50 grams or less. One 4 unit dose of Afrezza is usually adequate to cover these meals. But, it is not quite this simple. What about snacking and eating that occasional oversized meal filled with carb overload? This is when correction doses are necessary. As an example, here are a few of Spiro snacks that require 4 unit correction doses:
Pina Colada 50 grams 2 Mallomars 36 grams 3 Double Stuffed Oreos 33 grams 2 oz of Cheetos 26 grams Another Pina Colada 50 grams
In the past 70 days, Spiro has only used approximately 40 correction doses. In the old days, 5 month’s ago, Spiro would have watched a football game, downed a whole bag of Cheetos along with 2 non diet beverages and maybe a cinnamon roll to get rid of the Cheetos taste. That same day for a post dinner snack, Spiro would down 4 Mallomars and a glass of milk. Hey, that’s what fatso’s do.
Simply put, Afrezza may not be the best choice for a diabetic unwilling to take control of their diet and to reduce their carb intake to manageable levels. If money is no object, then a diabetic can eat away and down 24 -40 units of Afrezza per meal and it will work great, if you don’t mind weighing 300 lbs.
It amazes Spiro when he hears that some doctors are prescribing only 1 box of Afrezza with 90 doses per month. So what do their patients do about snack coverage?

Spiro, One of the healthiest sugar free snacks is nuts. They are loaded with protein, fiber, essential fatty acids and have a low glycemic load. About an ounce to ounce and a half of say almonds, walnuts, hazel nuts, pistashios and even peanuts (LIGHTLY salted but with no added oils) should do. MNKD's share price has fallen so low that Spiro had to get a new driver. The only thing that would make that picture better is if the "new driver" had a Dreamboat inhaler in its mouth too.

Dietary control is important for all diabetics, and pretty much everyone for that matter. Afrezza is still an insulin and eating more calories accompanied with more Afrezza will result in weight gain.

We are paying for my husband's Afrezza until our insurance drops, which we expect will happen within the next 6 months (January-February 2016). His company is with KaiserPermanete and they are reluctant to put anything on their plan before a year (we are told). WITH the coupon, two kits were about $575. He has had Type 2 diabetes for 8 years and his A1C had climbed, despite constant diet vigilance to almost 8. While one of his doctors was not bothered by this, we clearly were and changed our docs in order to get the script. No needles ever here!

That's when Afrezza will become very expensive for Type 2 Diabetics. Spiro expects he will fall into that big Donut hole in November. He is hoping that it is a glazed donut. Spiro here

You are exactly right, diabetics can select different unit mixes to get the needed dose. Spiro started Afrezza on 2/27 and he didn't start using correction doses until his cruise on 5/29. Spiro is still using his first box of 90 correction doses.
Spiro here, getting his prandial insulin exactly when his body needs it.

Nice one for oldeguy. Lost 10 pounds, maybe paying closer attention. (I had to think that through. glucose in blood, high blood glucose levels, water retention? An osmolar effect?) Type 2. Only on metformin before. A GP. Makes sense to me.
