I downloaded the color slide presentation. Fantastic! Page 13 introduces two new insulins: Toujeo and Afrezza.
Several pages of the presentation are devoted to Afrezza with emphasis on its speed and profile compared to the other bolus/prandial analogs, which are described in the presentation as NOT FAST ENOUGH.

Dr. Edelman also refers to Afrezza as Ultra Rapid Acting in his powerpoint presentation but he does not show the 12 unit. Also interesting to see on the page with the insulin pumps the Asante Snap pump. As of this past Friday, Asante closed it's doors and ceased operations. Some have speculated that the Afrezza / Toujeo combo will put pump manufacturers in a world of hurt. Much easier to do one injection / day and not have to mess around with calibrating the pump, infusion sets, etc and with a pair of pumps costing insurers north of $6500 (with the exception of Insulet's pump) the price of a Toujeo prefilled pen is hard to beat.

I learnt something new today. TCOYD founder is Dr Edelman. There's a bio on the latest newsletter. They are well supported by the big names throughout the industry. Both pharma and support networks. Certainly adds weight to anything he advocates. Nice find, thanks for sharing.

There is a caveat to your comments on direct-to-consumer marketing, though. Creating demand for Afrezza via DTC marketing without having first prepared and trained the physicians would have led to an unmitigated disaster. It's becoming more and more clear - to me anyway - how experienced Sanofi is at conducting product launches.
Some may argue with me, citing that Sanofi missed the importance of spirometers being available to endocrinologists, but I still am convinced that there is nobody better at this than our global giant of a partner.

Agree with MN. You just can't put the drug out there with out education of the Dr. and Sanofi seems to be doing a great job of doing just that with their training. We think its slow but it takes a lot of time and effort to get it right. A slow launch was the right thing based on training. You can't expect a busy Doctor to make a lot of effort to get the training until there is a need that is driven by his patient asking for a product that he knows very little about.

You are correct. I went in for a check up yesterday & my PA said she (Providence Medical) has not seen a SNY rep pitching Afrezza yet. She knows about it & will not hesitate to prescribe it to patients who will benefit from it. Her office also has a spirometer & she believes Afrezza will be a success in time. My understanding is the SNY reps dedicated to Afrezza are getting paid per script?

The importance of postprandial glucose control has long been overlooked. In type 1 diabetes,there is an unmet need for ultra fast-acting insulins to more effectively and efficiently control postmeal glucose values while simultaneously limiting delayed hypoglycemia. In addition to controlling postprandial glucose values, there is a need to treat incidental hyperglycemia between meals, which points to the demand for a rapid-on, rapid-off insulin. In type 2 diabetes, the endogenous ability of the pancreas to secrete insulin diminishes over time. Initiation of prandial insulin in type 2’s, typically after basal insulin has been added, is often a stepwise approach and, unfortunately, education and training for health care providers (HCPs) regarding how best to initiate and titrate prandial insulin has been broadly deficient or absent. With the advent and availability of new insulin preparations, including inhaled insulin, there is the urgent need for HCP education on all aspects of glycemic control with insulin-- including initiation, titration and ongoing adjustment.
In type 2 diabetes, there is also a marked behavioral resistance to advancing insulin therapy by both HCPs and patients. In particular, HCPs avoid intensifying mealtime insulin for a host of reasons and patients are often resistant to advancing insulin therapy primarily because of common misconceptions.
This workshop will feature concise, didactic presentations and will utilize an electronic audience response system (ARS) for interactive case-based learning. Importantly, the ARS questions will include clinical decision-making steps where multiple answers may be appropriate in order to facilitate discussions between faculty and participants and more effectively translate evidence to practice.
