4 days ago... “I am generally an avid reader and not a poster but I am so beyond upset. My daughter's endocrinologist prescribed Afrezza for her not quite 2 months ago. It has been THE MOST EFFECTIVE insulin she has ever used! She had reactions to Lyumjev and can't take it, Fiasp turned to water in a few months, and Humalog and Novolog take a minimum of 2 hours to kick in for her. We have mitigated a lot of her highs by looping the last few years but Afrezza has been a G*dsend for high carb snacks, sugary beverages, meals, and fun last minute things like funnel cake - just absolutely amazing for her. Today we received a letter from Express scripts that as of April 1, they will no longer cover it and she needs to switch back to Lyumjev or Humalog😭 I have calls pending with the doctor, and I just got off the phone with Afrezza assist. Afrezza assist can help us with an appeal, but they can't reach out on her behalf since it is off-label. It is incredible to me that more T1d's and Endos are not aware or prescribing this wonder drug. I plan to fight this to the 3rd and final appeal if necessary as there is nothing like it! If anyone has any advice on appeals or they have done this and fought Express Scripts, any information would be appreciated. Thank you.” Sugary drinks and funnel cakes? Why? Afrezza is fantastic at helping to control blood sugar levels but why (WHY!) would a type 1 eat these things? It defies common sense and does nothing positive for a Type 1's metabolism - Afrezza or no Afrezza. Final answer.

Sometimes kids want to be able to live a normal life like other kids and go to the fair and eat funnel cake, that’s my opinion. And Afrezza allows that. I probably shouldn’t even be posting these things on here..it’s a private page after all but I thought you guys would want to learn about what’s going on.

Mike has dangled the pediatric carrot in front of investors for 3 yrs now. If MannKind truly thought a pediatric indication would boost sales they would have submitted this indication to the fda years ago. Very few kids are going to be willing to inhale 6-8 times a day to cover their post prandiials. This is why MannKind hadn’t submitted to the FDA. Adults aren’t willing to dose Afrezza at the necessary frequency of inhalation’s. Again...another thing Mike can go to investment symposiums and sell as the future. You would think after going to the same investment meetings every year and raising capital EACH year on the same message SOMEBODY would start to question the message. However they do not. Keep investing in MannKind!!

Good ... thanks and I am investing a lot more in May. I always thought you were a long ... but, true colors always become evident. So, kids don't want to inhale 6-8 times a day and would rather be injected. Okay ... if you say so.

Back in the day, I saw what was a triangle or trifecta of blocking points for Afrezza adoption, imho, as follows:
A. PWD B. Endos C. Insurance
It seems to me that the PWD issue has been removed now that we are several years into its release and the diabetic community has had ample time to discuss among themselves. and the safety profile seems understood for the most part.
However, I still see B and C as blocking points to its adoption and this would include with Peds imho. I don't see why endos would change their minds for children. Actually, they might be even moe conservative. Second, insurance also has no incentive to change for children. This will proably occur despite what parents want.
For better or worse, I am resigned to feeling that Afrezza will likely never receive the acclaim or credit it so well deserves for being so spot on as a solution for PWD short of an actual cure for diabetes.
In the meantime, I also think Mnkd will keep it around for the time being as it does produce a steady $60 to $70m annually in gross revenue which when combined with Trep T royalties should begin to get mnkd to break even and a small profit until mnkd can figure out what is next. GLTAL's!!!!

I believe the first opportunity for significantly increased scripts will be from the Medicare Senior Savings Program which begins for Afrezza in just over 8 months from now. The cost will be only $35 per month out-of-pocket co-pay for Medicare seniors. If my memory is still good ... I remember that MNKD put out a list of doctors across the nation that were upstanding in their field and also known to prescribe Afrezza. Afrezza was very new and hardly anyone had heard about it. I remember that there was only one doctor listed on Oahu whose office was only about a mile or two away from me. I haven't checked lately, but currently many doctors know about it ... even my VA doctor knew about it but said that it wasn't on the VA's list of prescribable drugs so he couldn't prescribe it. But, that's mytakeonit I remember that list, but don't remember any claim that the docs were outstanding in their field. And I remember complaints by diabetics that when they went to one of these docs they couldn't get a script for Afrezza. The suspicion was that if a rep visited a doc, if only to drop off materials about Afrezza, the doc was added to the list because he or she then "knew about Afrezza." I think it is the same as the undisputed fact that since we bought MNKD shares we by definition are upstanding investors

I am optimistic that there will be a wide variety of Part D providers in the program for next year, based on the number this year which doesn't currently include Afrezza (bold emphasis is mine): Part D sponsors had the opportunity to apply for CY 2021 participation by May 1, 2020. CMS received significant interest in the Model from Part D sponsors, and has robust participation in the Model for CY 2021. Across the nation, 1,635 prescription drug plans, including both Medicare Advantage, Prescription Drug plans (MA-PDs) and standalone Prescription Drug plans (PDPs), are participating in the Part D Senior Savings Model for CY 2021. Seventy-six (76) Part D sponsors are participating in the Model for CY 2021, covering more than 13.8 million total enrollees. CMS estimates that the Model’s supplemental benefits will be directly available to more than 650,000 enrollees who use insulin and are enrolled in a participating plan for CY 2021. Seniors who use insulin in all 50 states, D.C., and Puerto Rico have a choice of a Part D Senior Savings Model participating plan in their area in CY2021.
